Covid Booster Vaccinations - our Federation (Hartlepool & Stockton Health) will be contacting patients to book appointments for their Covid Booster in the Town Hall, Raby Road very soon.  Patients will be contacted 6 months following their second Covid Vaccination.

PRESCRIPTIONS - Due to the problems we are all facing with Coronavirus (Covid - 19) the practice will accept prescription requests over the telephone, for the foreseeable future, for patients who have no access to the internet.  We have taken this action to reduce the risks to patients attending the surgery unnecessarily.  We also recommend patients nominate a pharmacy of their choice to enable us to send prescriptions electronically.

ONLINE ACCESS:  For Online Access please click on either of the links at the bottom of the Home Page which say:  "Order Your Repeat Prescription Securely Online" OR "Request an Appointment with a GP" - this will then give you the System Online Page where you key in your username and password we gave you.  Note: be very careful when keying is as it is case sensitive

Care Co-ordinator

The World Health Organisation defines health as "a state of complete physical, mental, and social well-being and not merely the absence of disease or infirmity." At Hartlepool & Stockton Health we are leading a pilot to provide extra, holistic support to patients who are living with long term clinical conditions which may put them at risk of frequent, unplanned hospital admissions.

Often known as “Social Prescribing”, our Care Coordination service helps patients to focus on the important social and mental factors that affect their health. Our 20 Care Coordinators work with every surgery in Hartlepool and Stockton, helping over 100 patients each week to create their own simple plan to enable them to feel better. We work with over 40 community and voluntary sector partners to help local people to stay safely and happily in their own homes.

​In a recent survey 27% of patients said that they felt the service was life changing and 69% felt their circumstances had vastly improved.

We are please to share with you all that the Care Coordinator Team have been recommissioned for a further 2 years.
